You can now complete your Self Employment Client Activity Reports online. Please note that all Self Employment forms are now in Adobe Acrobat PDF format. There are 3 ways to submit your report.

1. Online*. Complete the PDF form and press the submit button.
2. Offline**. Complete the PDF form, print it, then fax mail or deliver it.
3. Offline**. Print the form and fill out it at your convenience, then fax mail or deliver it.
